Freeze Membership

As per membership agreement, you may request a freeze for up to 3 months within a 12 month period. Your membership will be charged a one-time administrative fee of $40 each time your account is place on freeze and set to full rate on the month indicated. Medical freezes require a doctor’s note and no fee is charged. You must provide a doctor’s note to reactivate your membership. To request a freeze on your account, complete the form below, or complete a Membership Status Change Form at our service desk.
